			<content:encoded><![CDATA[<p>Repaglinide (Eurepa, Rrapilin), Nateglinide (Glintae, Natelide)</p>
<p>These drugs release insulin in response to meals and therefore regulate post-meal hyperglycemia (rise in blood glucose after meals) in adults. These should be avoided in liver disease. Repaglinide is ingested 10-30 minutes before meals. Side effects are mild headache, dyspepsia, joint pain, or weight gain. It can be given with other drugs. Usual dose of repaglinide is 0.25 mg to 0.50 mg before meals. Maximum 16 mg per day. Nateglinide is used in a dose of60 to 120 mg thrice a day before meals. Like pioglitazone it may increase blood cholesterol also.</p>
<p>Acarbose (Diabose 50 mg)</p>
<p>It inhibits enzyme which is present in small intestine. The enzyme normally breaks complex sugar of food into glucose. Therefore excessive rise in blood glucose does not occur after meals. Since it is not absorbed adverse effects are minimum (gases and loose stools).</p>
<p>Combined Drug Therapy</p>
<p>Since antidiabetic drugs lower blood glucose by independent and differing mechanisms, t~ese drugs are sometimes combined for better effect if one drug given alone fails to control the dIabetic state. Thus in secondary sulfonylurea failures, biguanides can be added. The following combinations are readily available:</p>
<p>The drugs given below have also been marketed for better control the diabetic&#8217;s blood sugar: Guargum (Carbotard, Diat-AIM) and Glucomannan (Dietmann) slow carbohydrate absorption by forming a get in gastro-intestinal tract. It reduces sudden rise in blood glucose after food intake. One sachet of glucomannan (1.2 g) or guargum (4 g) is to be taken before each meals. It can be taken with water, soup, or milk. Chromium picolinate (CAP CP 200, 400) increases insulin action on tissue as it acts as a co-factor for insulin action. It is used in doses of 200 ug to 500 ug per day.</p>

Elevated blood glucose doesn&#8217;t necessarily indicate diabetes, but it does call for more extensive testing. In general, a diagnosis is established if two separate blood tests, done after fasting for eight hours, show glucose levels of 140 milligrams of glucose per deciliter of blood (mg/dI). In borderline cases, a glucose challenge test may be ordered. This involves measuring glucose after fasting, and again after drinking sugar water.</p>
<p>Medical Treatments</p>
<p>There is no cure for either type of diabetes, but the disease can be controlled with a combination of therapies. People with Type I need regular insulin injections and a special diet and exercise regimen. They must also measure their own blood sugar . Most people with Type II diabetes can control it with just diet and exercise, especially if they lose weight. In fact, studies show that weight loss alone can produce normal blood glucose levels in more than 80 percent of overweight Type II patients. Others may need oral hypoglycemics to increase insulin production and its effectiveness. These drugs include chlorpropamide (Diabinese), glipizide (GlucotroI), glyburide (Diageta and Micronase), tolbutamide (Orinase), and tolazamide (Tolinase). Patients with both types of diabetes require extra medical care and should establish a close working relationship with a specialist, usually an internist or endocrinologist. Because diabetes affects especially the heart, blood vessels, kidneys, nerves, and eyes, other specialists may be needed.</p>
<p>For example, an ophthalmologist should be seen at least every 6 to 12 months to check for diabetic retinopathy, a disorder in which blood vessels in the retina overgrow and rupture. If unchecked, this bleeding can lead to blindness. Cardiovascular complications, such as high blood pressure and coronary artery disease, are particularly common in diabetes. A significant number of patients develop kidney failure, and require dialysis or a kidney transplant. Reduced circulation to the legs and feet may lead to the need for vascular surgery or even amputation. There is, however, good news for the 13 million Americans who have diabetes. A layear study showed that maintaining normal blood sugar levels dramatically slows the progression of complications. More doctors are now encouraging diabetic patients to follow a regimen that maintains blood glucose levels as near to normal as possible. This involves frequent glucose monitoring (at least four times a day), adjustment of insulin or other medication dosages, changes in exercise patterns,and alteration of the timing, frequency, md content of meals and snacks. To make insulin injections easier, :here is now an insulin pump, which is carried in a pocket or worn on a belt. It can be programmed to administer small amounts of insulin at specific times through a syringe left in place in the abdomen or other convenient site.</p>

Parents should assume this responsibility for young children; by age 9 or 10, most youngsters can use a self monitoring device. People with diabetes, especially children and teenagers, need emotional support and continual education to accept responsibility for their own care. Joining a group for diabetic patients or sending a child to a diabetes camp can make it easier to cope with the disease. A diabetes diet is similar to the healthful diets now recommended for all Americans: Obtain most of your calories from complex carbohydrates, such as vegetables, legumes, fruits, and whole grains, and rely less on animal foods for protein. Avoid refined sugars and restrict fat and cholesterol intake to help reduce the risk of heart disease.When diabetes is first diagnosed, you should consult with a registered dietitian to develop meal plans compatible with your tastes and lifestyle, and learn to follow the exchange lists developed by the American Diabetes Association. These lists provide simple formulas to assure that you get an appropriate number of calories and other nutrients each day. Eventually, you will not need to rely as heavily on them. In fact, with careful self testing, you can learn to increase or decrease your insulin dosage to accommodate an occasional departure from your usual diet. People with diabetes often have poor circulation and are susceptible to skin ulcers, especially on the legs and feet. Fit shoes carefully to avoid corns and other foot problems that can develop into a serious infection. Also keep toenails trimmed. If you have difficulty caring for your feet, see a podiatrist. Carry in your wallet identification indicating that you have diabetes so that you can receive prompt medical care if you are in an accident or have a high or low blood sugar reaction. Also, be alert for signs of an insulin overdose , and carry a source of sugar to use if one occurs.</p>
<p>Other Causes of High Blood Sugar</p>
<p>Certain medications can raise blood sugar. 50 too can diseases affecting the pancreas. Pregnant women sometimes develop gestational diabetes, which can be detected with a blood glucose test, usually in the second trimester.</p>

			<content:encoded><![CDATA[<p>People with diabetes are at higher risk for heart disease than non diabetics. People who have a cluster of metabolic disorders, characterized as the metabolic syndrome, are also at greater risk for heart disease. The underlying condition-insulin resistances present in both conditions but in varying degrees of severity. This chapter provides an overview of the characteristics of these metabolic disorders as well as advice on restoring insulin sensitivity and reducing the associated risks for heart disease and death.</p>
<p>Metabolic Disorders</p>
<p>Metabolic disorders occur when a specific enzyme or cofactor is absent or is present in insufficient quantities that result in the body&#8217;s inability&#8217; to receive particular nutrients. In other words, the body is unable to metabolize foods into nutrients for fuel or to build tissue. For people who have problems producing insulin, which is the key to the uptake of blood glucose (blood sugar), the body is unable to use the glucose that is circulating in the bloodstream for fuel. As a result, blood sugar levels become elevated.</p>
<p>If blood glucose levels are elevated because insulin is not available or insulin resistance is severe, the person is diagnosed with diabetes, which is a metabolic disorder. If, on the other hand, blood glucose levels are elevated because insulin sensitivity is impaired, and this impairment is present with other specific conditions, the person is considered to have a metabolic disorder, described as the &#8220;metabolic syndrome.&#8221; Both of these conditions increase the risk of heart disease. Many people with diabetes, including children and adults, also have lipid disorders, and many also have hypertension. Both of these conditions increase the risk of heart disease. Therefore, people with diabetes not only need to manage their blood sugar, but it is also important for them to manage their blood pressure and cholesterol carefully.</p>

<p>No one really knows what causes gestational diabetes The placenta supports the baby as it grows and hormones from the placenta help the baby develop. But these hormones also block the action of the mother&#8217;s insulin in her body. This problem is called insulin resistance. Insulin resistance makes it hard for the mother&#8217;s body to use insulin, so she may need up to three times as much insulin.</p>
<p>Gestational diabetes begins when your body is not able to make and use all the insulin it needs for pregnancy. Without enough insulin, glucose cannot leave the blood and be changed to energy. Glucose builds up in the blood to high levels. This is called hyperglycemia.</p>
<p>Gestational diabetes can be harmful to both the mother and the baby, so immediate care must be taken. Treatment always includes a specific diet plan, close blood sugar monitoring, and possibly even daily insulin injections. If your medication is not quite right or if the diet plan is not being followed correctly, there is a risk of hypoglycemia, the opposite of hyperglycemia. Hypoglycemia occurs when there is not enough glucose in the blood.</p>
<p>Gestational diabetes generally disappears after giving birth, but once you have had it, you have a greater chance it will surface again in future pregnancies. In some cases, gestational diabetes uncovers type 1 or type 2 diabetes.</p>
